Our practice accepts insurance for patients. To best serve you, please verify your insurance benefits before you arrive. Click on the link below and complete the online form. You will be contacted as soon as your benefits are established.

You must verify your insurance before you make your appointment. Please complete the form below and our team of experts will verify your insurance. This process may take a few days. Note: Please upload a copy of the front and back of your insurance card to ensure accuracy. If you are a NF or PIP patient, please have the claim number, date of the accident, the adjustors name, and number.

Once you have received approval from your insurance carrier, you will need a prescription from your primary care physician. You’ll need to explain your specific symptoms.  Various emotional and physical conditions qualify for massage therapy and bodywork. Stress-related symptoms, circulation issues caused by diabetes or hypertension, arthritis, frozen shoulder, fibromyalgia, anxiety, depression, and chronic back pain are all examples that could qualify for massage therapy.

 Please upload all documentation to the portal for the verification process.  Your prescription will need to include the following:

• Medical condition or injury (the reason as to why you need the massage/s)

• The number of sessions you will need (once per week, twice per week, etc.), and duration, i.e., 60min. Please also verify this with your insurance carrier.

• Any and all area(s) which need to be addressed.

• If applying with health insurance you will need strong out-of-network benefits that specifically cover massage therapy or manual therapy that allows a Licensed Massage Therapist.

• By submitting this form, the patient understands they are fully responsible for any fees not covered by their insurance and will pay all bills in full by their credit card on file.
